Anyway, Medical Negligence I Have Known (Instalment 185973):

Registrar: "[Minister], I want to talk to you about when you were anaemic in 2004."

Minister: "Er, I wasn't aware I was anaemic in 2004."

"Yes, it's in your notes."

"Oh.  OK.  Nobody told me."

"Dr. Callam didn't tell you?"

"Who's Dr. Callam?"

"He's your GP.  Or at least he was in 2004."

"No, he wasn't.  Unless he was a locum I don't think I've ever seen a Dr. Callam."

"You've seen him a number of times."

"No, I haven't."

[pause]

[pause]

[registrar tits about on computer for a moment]

"Ah.  That explains it."

"What?"

"I'd put the wrong hospital number in and was looking at the wrong notes."

"Right you are…"

"Oh, yes: this makes much more sense."

"Good."

"But you haven't had many of the blood tests I would have expected."

"Er, I've had more blood taken this year than when I was donating blood four times a year."

"Have you?"

"Yes."

"This year?"

"Yes.  At one stage earlier this year I had track marks."

[pause]

[pause]

[registrar tits about on computer for a moment]

"Ah.  That explains it."

"What?"

"For some reason you've got three different hospital numbers and your test results are spread across the three different sets of notes."

"But of course…"
